- Business Address: The full address, including suite or building numbers, where the beer and wine will be sold.
- Corporation Names: Have at least 3 potential business names (e.g., 123 Store LLC, New 123 Store LLC). If you already have a corporation, bring the formation documents and your EIN.
- DBA Name: The “Doing Business As” name for your business.
- Contact Information: A reliable mailing address, email address, and cell phone number.
- Residential & Employment History: Addresses where you’ve lived for the past 5 years, and employers’ details from the same period, including job titles and dates.
- Spouse Information (if applicable): We will need your spouse’s full legal name, Social Security Number, Driver’s License Number, and Date of Birth. Additionally, let us know if your spouse owns a liquor store or has had any legal or T.A.B.C. issues.
- Other Household Members: If anyone over the age of 18 lives with you, we will need their full legal names, Social Security Numbers, Driver’s License Numbers, and Dates of Birth. Let us know if they own a liquor store or have had any legal issues related to T.A.B.C.
- Proof of Legal Status: Bring your Passport, Naturalization Certificate, or Green Card.
Additional Considerations:
Before applying, check if your business is near any of the following:
- Churches within 300 feet
- Day Cares within 300 feet
- Public or Private Schools within 1,000 feet

- Lease Agreement: A copy of your lease, the landlord’s EIN, and the Date of Birth of the corporate officers who own the property. If you’re purchasing the land, provide details of the purchasing corporation.
